PROVIDER REQUIREMENTS: HPC, HPC transportation
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Mild ID, Carpal Tunnel, Arthritis, Depression, Bipolar DO; OCD; Pre-Diabetes, Hypertension.
HOUSING: Housing in Madison with family
DAY PROGRAM/EMPLOYMENT: Attends VOC 3x week
