PROVIDER REQUIREMENTS: Med Cert 2 This individual requires total support due to seizure disorder, cerebral palsy, Spastic Quadriplegia, Non-Verbal, Microcephaly, G-tube feedings.
TRANSPORTATION NEEDS: modified vehicle.
