DESCRIPTION OF INDIVIDUAL: Individual is a 23-year-old female who is looking for an ADS program to pick her up after Voc hab (anytime between 2pm-2:40ish) and transport her back to their ADS program until her family picks her up around 430 pm. She is very social, loves games, movies, tv, and engaging in any group activity. She does struggle with impulse control and will eat any food laying out (no diagnosis attached to this) if nobody is paying attention.
PROVIDER REQUIREMENTS:
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Autism, ADHD-Combined Type, Generalized Anxiety Disorder, High Cholesterol and epilepsy.
HOUSING: This individual lives in an apartment in Willowick Ohio with her parents.
DAY PROGRAM/EMPLOYMENT: Attends a Voc Hab during the day in Mentor.
