DESCRIPTION OF REQUESTED SERVICES: Individual is a 67-year-old woman who is looking to attend a Vocational Habilitation program for 3 days/week and an ADS program 2 days a week, with transportation to both. She is familiar with work tasks that include piece work and sorting. She enjoys watching tv, doing crafts and going shopping.

DESCRIPTION OF INDIVIDUAL: This individual recently moved to Lake County, after spending a length of time in a nursing facility in Cuyahoga County. She has not worked in several years but is interested in attending a voc. hab. program to earn a paycheck. She would also like to attend an ADS program to have opportunities to go into the community, engage in preferred activities with peers and do leisure activities. This individual has several medical diagnoses, including Schizophrenia, which have caused her to become aggressive in tone (not physically aggressive) and states that she is hearing voices in her head. There are details for responding to these “episodes” outlined in her ISP. She is ambulatory and requires a non-modified vehicle for transportation. She requires auditory supervision in familiar environments and visual supervision in the community. She is able to use the restroom, dress and eat meals independently.
PROVIDER REQUIREMENTS: Provide services as written in the ISP.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Mild ID, Schizophrenia, Asthma, Depression, Hypertension and history of seizures.
HOUSING: Lives in apartment in Willoughby Hills, OH.
