DESCRIPTION OF REQUESTED SERVICES: Individual is in need of Skilled Nursing level of care (Medication Certification Level 2 – G tube feedings). The family has two potential “shifts” they are looking to fill: 8:00 am-5:00 pm daytime shift while the individual attends school; 12:00 am – 8:00 am overnight shift while the individual sleeps. He requires Total Assistance for all ADLs; all medications, nutrition, and fluids must be administered by certified staff. There is also a seizure protocol and rescue medication plan in place.

DESCRIPTION OF INDIVIDUAL: Individual is a 19 y/o male who is affectionate with his family. He is a pre-linguistic, non-verbal communicator and communicates with others through grunts/coos, limited facial expressions, and will eye-gaze between 2 icons/choices. He can follow objects with his eyes and enjoys listening to music.
Additional adaptive equipment utilized includes a wheelchair (with pelvic belt, anterior chest support, ankle huggers, and toe straps on wheelchair), Stander, Ankle-Foot Orthotics (AFO), positioning wedge, shower chair, and in-home ceiling lift system. Due to constant “athetoid-like movements”, he benefits from supportive positioning with strategic strapping and sensory supports to facilitate trunk control, head control, and functional upper-extremity use.
PROVIDER REQUIREMENTS: Medication Certification Level 2
TRANSPORTATION NEEDS: Modified vehicle.
DIAGNOSES: Down Syndrome, IDD-Profound, Congenital Quadriplegia, Epilepsy (“West Syndrome”), Chronic Lung Disease, Hypothyroidism, Hyponatremia, and Sleep Apnea. All nutrition/fluids are administered via G-tube.
HOUSING: Individual lives with his parents in Mentor.
DAY PROGRAM/EMPLOYMENT: Currently attends LEEP at Broadmoor.
