DESCRIPTION of NEEDED SERVICES: Individual needs Vocation Habilitation Program 2 days per week on Monday and Thursday. Attends a Day Program the other 3 days during the week, although he may ultimately decide to attend Voc Hab Program all 5 days.

DESCRIPTION of INDIVIDUAL: A 41-year-old male who is caring and has a great personality and smile but has difficulty with effective communication, which can cause occasional agitation. He has a communication device to assist with communication but needs to be reminded to use it. He worked in the community without supports until recently, but the team is concerned that his actual work skills have not developed in that setting. He is interested in making money. He’s very pleasant and agreeable and willing to try new things. He does like attention from non-peers and can become temperamental when he does not get his way. Likes watching WWE, loves music and dancing, and activity in the community.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Down Syndrome, high cholesterol and acid reflux. Allergies: Augmentin (severe), dust, cats, pollen, smoke
HOUSING: Lives in congregate setting in North Mentor.
DAY PROGRAM: Tues, Wed, Fri on Tyler Blvd.
SUPPORTS NEEDED: Vocational Habilitation Program with ample work and income opportunities.
