DESCRIPTION of INDIVIDUAL: This person is a friendly 23-year-old man who is verbal and requires 24-hour supervision and requires total assistance into the community. No alone time. He lives in Madison with his dad and enjoys being active. He wants to be very active in the community and enjoys Deepwood events. Must have reliable vehicle and safe driving record. Minimal assistance with hygiene is mostly verbal reminders and prompting.
PROVIDER REQUIREMENTS:
A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Attention Deficit Hyperactivity Disorder, Oppositional Defiant Disorder, Reactive Attachment Disorder, unspecified intellectual disabilities.
HOUSING: Lives in Madison with dad
