DESCRIPTION OF REQUESTED SERVICES: Individual is looking for Residential Respite services to access the community 2-3x per week; likely Mondays, Wednesdays and Fridays.

DESCRIPTION OF INDIVIDUAL: This individual is 46 years old and lives in Mentor with family that provide Shared Living support to him. This individual is able to communicate verbally and has strong cognitive skills. The individual utilizes a manual wheelchair to navigate his environment and is able to use a slide transfer board independently. He does need to reposition from his wheelchair every 5 hours to avoid pressure ulcers. This individual is able to self-medicate and independently uses a catheter. This individual loves reading and learning and is eager to make friends. He enjoys watching the news and loves the Cleveland Guardians and the Chicago Bulls. This individual would like to access the community to exercise and increase his opportunities to be active.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Wheelchair accessible vehicle is needed OR a provider that is willing to ride Laketran with the individual
DIAGNOSES: Spina Bifida and congenital hydrocephalus
HOUSING: Individual lives with family in Mentor
DAY PROGRAM/EMPLOYMENT: Individual is not working or attending a day program at this time
