DESCRIPTION of NEEDED SERVICES: Front entry way modifications to include restructuring the rampway to make it ADA compliant. Widening doorway and entryway to allow for an adult size power wheelchair to enter the doorway and into the house safely and independently. A comprehensive PT assessment has been completed and will be shared with interested providers. These modifications are for a 14-year-old boy who lives with his family in Willougby Hills.
