DESCRIPTION OF INDIVIDUAL: 61-year-old male who always utilizes a manual wheelchair. He is encouraged to wear his Halo head brace or helmet especially when doing any kind of transition (bathroom, vehicle, wheeling in community) as he is a larger male who is prone to falls due to weakness in his body. He is extremely friendly and prides himself on work and likes to stay busy. He is hard to understand at first due to severe mumbling but those around him daily pick up fast. He loves to be around people and is a big jokes guy.
PROVIDER REQUIREMENTS: N/A.
TRANSPORTATION NEEDS: There are some mobility issues. A wheelchair accessible van would be great but can, if necessary, ride in a non-modified vehicle as he does at home.
DIAGNOSES: Moderate Intellectual Disability, borderline hypertension, muscle spasms, hay fever, dermatitis, allergic rhinitis, seizure disorder, bi-polar disorder, depression, OCD, chronic tenia pedis, syncope hyperlipidemia, anxiety.
HOUSING: Lives in Mentor.
