DESCRIPTION OF REQUESTED SERVICES: Individual is requesting HPC/respite services. Currently the individual is on the HCBS Waiver waitlist and will be utilizing county Family Services and Supports (FSS) funding to pay for HPC services. Since FSS funding is limited, HPC services will be requested at limited hours and called “respite” for FSS billing; the family is flexible on the schedule while she is on the waitlist. However, once the waiver is allocated, the individual will need regularly scheduled HPC services. When she was enrolled on a waiver in another state, the individual received 80 hours/week, 7 days/week of HPC services with an additional 16 days of respite.
While in the home, the individual will need supervision and occasional physical assistance to prepare/cook a meal. She may need verbal prompts up to physical assistance with some household chores. She requires a verbal prompt to remove a jacket or sweater if she begins to overheat in warm weather. When she uses the restroom, she needs a verbal reminder to wipe herself and to wash her hands.
The individual recently moved to Ohio after her mother passed away. She is unfamiliar with the community and would enjoy exploring various activities and programs in the community with HPC staff. While in the community, the individual can freely ambulate but prefers to hold a staff member’s hand when crossing the street. She has limited safety / “Stranger Danger” skills and needs visual supervision in the community to ensure her safety. Provider will need to be aware of individual’s triggers (e.g. loud noises, strong smells, getting her clothes wet) and help individual avoid triggers when possible. When individual is triggered, she responds by yelling, crying, pushing, attempting to flee the area, or she will disrobe whenever her clothing is wet and touching her skin. Staff will need to help her regulate safely or provide her with a change of dry clothing.
In all settings, the individual will need verbal prompts up to physical assistance for regular blood glucose monitoring tests, insulin injection, and potential emergency medications due to “Brittle Diabetic” condition. Her glucose levels can have intense and unpredictable swings. Individual’s blood glucose levels are tested up to 20x/day. Should her glucose levels drop and individual loses consciousness, staff will need to call 911 to have individual transported to the hospital.

FUNDING SOURCE: FSS funding while on the HCBS Waiver waitlist. Once the waiver is allocated, HCBS waiver funding will be utilized.

DESCRIPTION OF INDIVIDUAL: The individual is very kind, compliant, soft-spoken, and eager to help others. She loves the cats in her dad’s home and has been eager to help feed and care for the cats. She enjoys swimming and going to the YMCA. She also enjoys going to the Natural History Museum.
Additional information to know about the individual to provide more sensitive, trauma-informed care: The individual lived with her mother and sisters her entire life. Her mother unexpectedly passed away in September 2025. The individual has a different father than her sisters; she moved to Ohio to live with her father and stepmother, who previously had limited contact with the individual. She is still adjusting to her mother’s death, the extreme changes to no longer living in her home, not having daily contact/interaction with her sisters, not receiving daily services from her known team of waiver providers, and navigating an entirely new household/routine. Her father was also just diagnosed with a significant health issue and requires ongoing treatment outside of the home to address his health, creating even more change in the household/routine.
PROVIDER REQUIREMENTS: Med-Cert level 3 for Insulin injections.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Autism Spectrum Disorder; IDD-Mild (FSIQ 60); Diabetes Mellitus Type 1 – Insulin Dependent *BRITTLE DIABETIC*; Hashimoto’s Disease; Celiac Disease (can’t consume gluten); Generalized Anxiety Disorder; Major Depressive Disorder with seasonal pattern.
*History of seizures but has not had a seizure since 2022; not prescribed any anticonvulsant medications.
HOUSING: The individual lives with her father and stepmother.
DAY PROGRAM/EMPLOYMENT: None yet; however, the individual may be interested in beginning ADS once waiver enrollment is complete.
