DESCRIPTION OF REQUESTED SERVICES: Individual is looking for a day program to attend 2-3 days/week. The individual prefers a quieter environment and enjoys spending her time at program and does not prefer to go on outings/into the community.

DESCRIPTION OF INDIVIDUAL: This individual is a 53-year-old female who enjoys spending her time sitting under her blankets, watching DVD’s and relaxing. She needs total assistance with ADL’s, medication administration, mobility (she will crawl) but most of the time she utilizes her wheelchair.
PROVIDER REQUIREMENTS: Provide all services indicated in the ISP.
TRANSPORTATION NEEDS: Modified vehicle.
DIAGNOSES: Profound Intellectual Disability; Seizure Disorder; Cerebral Palsy; Spastic Dystonia, Depression, Anxiety, Seasonal allergies, history of constipation; history of low blood pressure and Hypothyroidism. Amy has occasional Urinary Tract Infections.
HOUSING: This individual lives with one other individual in a home in Wickliffe.
DAY PROGRAM/EMPLOYMENT: Currently attends a program 5 days/week.
