DESCRIPTION OF REQUESTED SERVICES: Individual is looking for HPC services for approximately 10-12 hrs/week starting in late June for community engagement and participation in recreation activities. Weekly M-F schedule is flexible with one weekend day set based on conversation with client and guardian.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Either agency or independent provider, no preference

DESCRIPTION OF INDIVIDUAL: Individual is a 24 y/o female. She is verbal but can be difficult to understand. She does not use a communication device. She attends a day program 5x/wk. She enjoys having access to her sensory toys. She enjoys playing on her tablet and watching YouTube videos. She loves going to dances, out to eat, and to the movies.
PROVIDER REQUIREMENTS: Med certified (needs PA with inhaler, PRN)
TRANSPORTATION NEEDS: non-modified vehicle
DIAGNOSES: Moderate ID, ADHD, general anxiety disorder
HOUSING: At her family home in Painesville, OH
DAY PROGRAM/EMPLOYMENT: Grape Escape in Geneva, OH M-F
