DESCRIPTION of NEEDED SERVICES: 50-year-old male is looking for an ADS program to attend outings within the community, 2 days per week. Staff to provide verbal and visual supervision while in the community.

DESCRIPTION of INDIVIDUAL: Social, kind, and energetic 50-year-old male who resides in a Wickliffe with his mom. He enjoys keeping busy and meeting new people.
PROVIDE REQUIREMENTS: Provide CI services, including visual and verbal supervision in the community.
TRANSPORTATION REQUIREMENTS: Individual is mobile and can transport in a non-modified vehicle.
DIAGNOSES: Mild intellectual disability, Tourette’s, generalized anxiety disorder, OCD, acid/esophageal reflux, hiatal hernia.
