DESCRIPTION OF INDIVIDUAL: Individual is a male in his early thirties. Individual lives in Kirtland, Ohio. There are two dogs and a cat in the home. Individual has a good sense of humor and is easy going. Individual is able to direct his own care. Individual is on a vent at night. Night shift tasks needed are- repositioning, suctioning, and monitoring of ventilator.
PROVIDER REQUIREMENTS: Provider must be an RN or LPN. Provider must be trained on trache/vent care.
TRANSPORTATION NEEDS: Modified Vehicle
DIAGNOSES: Scoliosis, congestive heart failure, vent at night.
HOUSING: Kirtland, OH
