DESCRIPTION OF INDIVIDUAL: 21-year-old, male looking for voc hab program. He is soft spoken and will communicate when prompted. He enjoys books, playing on phone, and shopping. He is a nice guy, and close to his family. He would like to engage with others and build friendships with others around his age.
PROVIDER REQUIREMENTS: Provide assistance with learning job skills, and assistance with communication and building rapport with others. Assistance with completing task and engaging with others. Need assistance with decision making. General supervision.
DIAGNOSES: Dandy Walker Syndrome, 22q.11.2 deletion, other anomalies of the brain and a FSIQ of 54.
