DESCRIPTION of NEEDED SERVICES: Individual had an “Open Sesame” automatic door opener installed several years ago that is need of service.

DESCRIPTION of INDIVIDUAL: A 55-year-old female with cerebral palsy who lives in her own home in Madison. She has a wheelchair and utilizes the “Open Sesame” for independent access in and out of her home. The agency who installed the technology is not available to inspect/service the door opener. She is smart and independent and relies on technology to help her maintain her independence.
PROVIDER REQUIREMENTS: A provider who is certified to install and repair Assistive Technology Equipment.
DIAGNOSES: Cerebral Palsy and wheelchair dependent.
HOUSING: Lives alone in her own home in Madison.
DAY PROGRAM: None currently.
SUPPORTS NEEDED: Assistive Technology Equipment or Repair
