DESCRIPTION OF REQUESTED SERVICES: Individual is looking for NMT transportation 5x per week to and from her home in Kirtland and her day program in Painesville. Individual needs dropped off between 8:45 -9:15 and picked up between 1:30-2:00. Individual has had behaviors in larger transportation, 1:1 transport is preferred, but transportation where the individual can have preferred seating out of arms reach from other individuals would be sufficient. Individual typically brings two backpacks to her day program so room for these bags is also needed.

DESCRIPTION OF INDIVIDUAL: Individual is a 29-year-old female with Downs Syndrome. Individual lives by herself in a manufactured home in Kirtland. Her personal items are very important to her and she values them greatly. Individual is soft spoken but is typically kind but can occasionally have bad days, this usually results in the harassment of peers.
PROVIDER REQUIREMENTS: NMT.
DIAGNOSES: Down Syndrome, OCD, and Depression.
HOUSING: Manufactured home in Kirtland.
DAY PROGRAM/EMPLOYMENT: Individual attends day program 5x per week.
