DESCRIPTION OF INDIVIDUAL: 36-year-old female, lives in mentor with her parents. She attends ADS five days a week. She is very friendly, enjoys crafting and doing art. She enjoys having a routine, requires verbal prompts and assistance with meals, and snacks. She enjoys looking through books, and magazines. She has rituals she does when she is upset or worried. She squeezes sensory items and will change her clothes or sweatshirt. She wears her sweatshirts even when it is hot outside. She does not like loud noises or being rushed, and she has difficulty with routine changes. She has no alone time at home or in the community
PROVIDER REQUIREMENTS: Residential Respite HPC certified. Provide supervision, verbal prompts and reminders of routine, assist with restroom, and personal hygiene, physical assistance with large curbs and steps on community outings.
DIAGNOSES: Moderate Intellectual Disability, Anxiety Disorder, Rheumatoid Arthritis, and Renal Tubular Acidosis. Seizure diagnosis.
HOUSING: Lives in family home.
DAY PROGRAM/EMPLOYMENT: attends ADS.
