DESCRIPTION OF INDIVIDUAL: 38-year-old, limited verbal ability, mostly receptive to spoken direction and information. Enjoys being up and moving, loves helping out when he sees it and will approach complete strangers to help them load/unload heavy things. Very friendly and easy-going.
PROVIDER REQUIREMENTS: Provide assistance with accessing the community and opportunities to help others, learn daily living skills, and build social skills.
DIAGNOSES: Moderate ID, Sx disorder, bilaterial hearing loss.
HOUSING: With family
DAY PROGRAM/EMPLOYMENT: Attends VH, looking to possibly split time between programs.
