DESCRIPTION OF INDIVIDUAL: Individual is a 53-year-old female. She is social and will want to chat about her day. She enjoys magazines, puzzles, word searches, and coloring. She is very helpful and loves her family and animals.
PROVIDER REQUIREMENTS: Provide assistance with learning job skills, and assistance with communication and building rapport with others.
DIAGNOSES: Mild ID, Epilepsy, Anemia, Borderline Hypothyroidism, and Anxiety/Panic Attacks
HOUSING: N/A
DAY PROGRAM/EMPLOYMENT: Group employment M-F.
