DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: This person is a friendly 23-year-old male with a good sense of humor. He resides with his parents in Willoughby and works at Breckenridge Rehabilitation. This young man requires a smart calendar called Skylight Calendar 2 to assist with being more independent with scheduling of appointments and work schedule.
