DESCRIPTION OF REQUESTED SERVICES: Female client seeking Residential Respite hours with transportation. Would prefer a female staff member. Avg 8-12 hours weekly.

DESCRIPTION OF INDIVIDUAL: 23-year-old female who lives in Concord with her parents and brother. This individual can communicate verbally but has a delayed response and will repeat what you are saying. She has reading and writing skills. She can complete most grooming tasks with verbal prompts and can ambulate independently. She requires auditory supervision at home and visual supervision in the community. She may need to be re-directed from grabbing, pushing, and does sometimes throw items. She has OCD and one of the tics she displays is frequent somersaulting. She will typically do this no matter the environment she is in and does not review if her surroundings are safe first. She benefits from reminders to twirl in place when somersaulting is observed. She is navigating medical appointments and will benefit from someone willing to be consistent, engaging, and understanding. She has a variety of interests including listening to music and singing. She likes plush dolls, Disney Princesses and female characters. Her increase in medical issues has led to an increased attachment to her mom. To reduce separation anxiety the family would like to explore introducing her to new, trusted people. To start, hours will be provided in the family home.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.

HOUSING: Lives in Concord with family- Mom is shared living provider.
DAY PROGRAM/EMPLOYMENT: Connected with a program, does not consistently attend (Avg 1x per month)
