DESCRIPTION of NEEDED SERVICES: Two women who live in a home in Madison need modifications to one of their full bathroom. The modifications include a roll in shower that is reoriented in the bathroom to allow larger shower space. Grab bars and proper sloped floor will need to be included. PT recommended a wet room style bathroom. A reclinable PVC style shower chair with brakes, seat belt and opening for peri-care is needed as well. Also, in this bathroom a high commode with grab bars is recommended. PT assessments will be provided to interested providers prior to quote visit.
