DESCRIPTION of NEEDED SERVICES: The individual is a 38-year-old male who lives in Mentor. Looking for a few days of residential respite at his home in Mentor while his father (not a paid provider) is not able to be home with him. The exact dates are not known at this time, the family would like to understand the process and meet with some providers before scheduling this time away. The individual is diagnosed with Down syndrome and eczema

DESCRIPTION of INDIVIDUAL: The individual is verbal but can sometimes be hard to understand. He does not always use complete sentences and may say yes to questions that are not yes or no questions. He does best with a few choices. He is independently mobile, able to feed himself, and goes to the bathroom independently.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. This individual is looking for assistance with supervision, laundry, bathing, changing clothes, medication administration, and meals.
TRANSPORTATION NEEDS: Modified vehicle.
DIAGNOSES: The individual is diagnosed with Down syndrome and eczema.
HOUSING: This individual lives with his father in Mentor, Ohio
DAY PROGRAM: The individual goes to Willoughby DI during the weekdays M-F
