DESCRIPTION of NEEDED SERVICES: This individual is a 36-year-old male who lives in Willoughby with his mother. He is looking for a day program to attend with non-medical transportation part time for socialization with his peers. He would be interested in attending 3-4 days per week.

DESCRIPTION of INDIVIDUAL: PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: TBI, S/P stroke, epilepsy
HOUSING: He lives at home with his mother.
DAY PROGRAM: He does not currently attend a day program.
SUPPORTS NEEDED: He would need assistance with different activities to participate in whether at the program or in the community. He would also need assistance with steps/curbs/uneven terrain.
