DESCRIPTION OF REQUESTED SERVICES: This individual is a 7-year-old female who resides in Painesville, OH. The individual’s family is seeking HPC to assist the family in increasing the individual’s engagement in community activities as well as assist with ADLs and maintaining the individual’s safety in the home. They are looking for approximately 12 hours per week.
SEEKING AGENCY OR INDEPENDENT PROVIDER: Open to either independent or agency providers.

DESCRIPTION OF INDIVIDUAL: This individual is non-verbal and primarily communicates through facial expression, body language or gestures. Individual has an AAC device but is still working to make the connection between icons and their real-world counterparts. This Individual loves high fives, clapping, hugs and tickles when she has done something positive. This Individual enjoys being outside, painting, books, her saucer swing, bouncy ball, watching puppy Pals on her tablet, Swimming and playing on the trampoline and slide. Individual is independent with mobility and is very active. Individual has no awareness of danger and will elope in public, school and community. Individual engages in self-injurious behaviors and utilizes a safety helmet as needed to ensure her safety. Individual requires visual supervision while in the home, close and constant supervision when in the community.
PROVIDER REQUIREMENTS: Training in Crisis Prevention and Intervention techniques is preferred but not required.
TRANSPORTATION NEEDS: Non modified vehicle.
DIAGNOSES: Autism, Obstructive Sleep Apnea Syndrome
HOUSING: Individual resides in her family home. Services will primarily occur in the home.
DAY PROGRAM/EMPLOYMENT: Individual is of school age.
