DESCRIPTION OF REQUESTED SERVICES:
Looking for Residential Respite: 3-4 hours on M, W, F, and Sunday’s ideally to increase access to and participation in community activities while ensuring health /safety needs are met. Individual uses a wheelchair for mobility and requires transportation in a modified-wheelchair accessible vehicle for community outings. Requires continuous supervision to maintain health and safety and assistance with all activities of daily living and mobility. Individual requires a physically capable and experienced provider who can safely perform transfers and provide hands on assistance throughout the duration of services.

DESCRIPTION OF INDIVIDUAL: This individual is an adult male who enjoys car rides and watching movies/shows. He has a great big smile and is nonverbal, and relies on facial expressions, gestures, body language, and other nonverbal cues to communicate his wants, needs, and preferences.
