DESCRIPTION OF REQUESTED SERVICES: Individual is looking for HPC services on Mondays, Tuesdays, or Wednesdays. The hours are flexible but averaging 8-12 hours every week. The hours would be after day program. The hours would 1:30/2:00-6:00 or 2:30pm-6:30pm in Mentor.

DESCRIPTION OF INDIVIDUAL: Individual is a 22-year-old male living in Mentor with his mom and shared living provider. Individual has a behavioral add on. Individual may become frustrated when he needs help or does not like something. He does enjoy being active. He enjoys cooking, being outside, collecting sea glass, and loves sports cards.
PROVIDER REQUIREMENTS: HPC, HPC transportation, and/or Residential respite
TRANSPORTATION NEEDS: Non-Modified vehicle
DIAGNOSES: Autism, Anxiety, and OCD
HOUSING: At his family home in Mentor
DAY PROGRAM/EMPLOYMENT: Brighter Horizons
