DESCRIPTION of NEEDED SERVICES: This individual is looking for a male HPC provider for late afternoons/evenings.

DESCRIPTION of INDIVIDUAL: This is a 25-year-old male individual. He loves sports, going to the YMCA, working at his job in the community, and playing video games.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
DIAGNOSES: Intellectual Disability
HOUSING: Individual lives on his own, in Mentor, Ohio.
DAY PROGRAM: Has a job in the community and attends AMP Services LLC. 1-2x/week.
SUPPORTS NEEDED: Tony needs assistance with activities of daily living, review/reminders of community safety, safety skills in general, boundaries, etc.
