DESCRIPTION OF REQUESTED SERVICES: Transportation is needed from the individual’s home to and from her place of employment at Giant Eagle (Mentor-on-the-Lake location), which is approximately 2 miles from her residence. Transportation is currently needed on Sundays, Mondays, and Thursdays. Her scheduled work hours are 12:00 p.m. to 5:00 p.m.; however, she has requested a schedule change ending at 4:00 p.m., which has not yet been approved or confirmed. The individual HPC provider may be available to assist with transportation on Sundays when needed.

PROVIDER REQUIREMENTS: NMT up to 6 trips per week for work
TRANSPORTATION NEEDS: Non-modified vehicle
DIAGNOSES: Mood Disorder, NOS, Mild Intellectual Disability, Fetal Alcohol Syndrome, Acid reflux, Migraines, Growths on Liver; Rheumatoid Arthritis and Gout.
HOUSING: Individual resides in an apartment in Mentor-on-the-Lake.
