DESCRIPTION OF INDIVIDUAL: Individual looking for a new provider for a fresh start. He would like the chance to make new friends and meet new people. Individual needs assistance with money management (has a provider), dietary assistance/education, and med administration. Individual can complete ADLs appropriately and has up to 2 hours of home/community alone time with guardian approval. He enjoys sports, casinos/gambling, spending time in the community, music/concerts, helping others and feeling valued and appreciated. He enjoys working and money is very important to him.
PROVIDER REQUIREMENTS: Med Cert I
TRANSPORTATION NEEDS: Individual does not require a modified vehicle.
DIAGNOSES: Tourettes, Depression, Anxiety and Intermittent Explosive Disorder
HOUSING: Willowick. Individual would also like to look at available homes.
DAY PROGRAM/EMPLOYMENT: Group employment services 4 days/week, vocational program 1 day/week.
