DESCRIPTION OF REQUESTED SERVICES: Male 22-year-old Individual is looking for a provider 1x/every other week, or 1x/month to help integrate into the community and play sports (pickleball, basketball, softball).
SEEKING AGENCY OR INDEPENDENT PROVIDER: Agency or Independent provider.

DESCRIPTION OF INDIVIDUAL: Individual enjoys being busy, going on vacations, going to sporting events, playing in sporting events and enjoys working his job 3 days/week.
PROVIDER REQUIREMENTS: Staff would need to follow service/supports in the ISP.
TRANSPORTATION NEEDS: Individual would need a non-modified vehicle.
DIAGNOSES: Moderate intellectual disability, Klinefelter Syndrome, Uses testosterone replacement patch daily, Tree nut allergy- prescribed EpiPen, Astigmatism, prescribed eyeglasses effective 07/09/2025.
HOUSING: He lives with his parents in Mentor, Ohio.
DAY PROGRAM/EMPLOYMENT: RedHawk 3x/week, second shift.
