DESCRIPTION OF REQUESTED SERVICES: Individual is looking for a HPC provider for congregate setting. The individual currently lives by himself but is searching for a housemate. The home is staffed 24/7 with OSOC supports overnight except for when the individual attend ADS 3x per week. Individual is nonverbal and needs assistance with all ADL’s. Individual ambulates with a gait belt and needs staff assistance with this. Individual is non-verbal.

DESCRIPTION OF INDIVIDUAL: Individual is nonverbal and needs assistance with all ADL’s. Individual ambulates with a gait belt and needs staff assistance with this. Individual is non-verbal. Individual requires physical assistance with all routine and heavy cleaning. He enjoys going in the backyard when the weather is nice, watching TV (Animal Planet, construction shows, sports) and movies especially Elvis.
PROVIDER REQUIREMENTS: Med Cert 1 required.
TRANSPORTATION NEEDS: non modified vehicle.
DIAGNOSES: Profound intellectual disability, Behavior components of Autism, prone to swimmer’s ear, unspecified epilepsy, Onychomycosis, and short heel cords. He has seasonal allergies and his adaptive equipment includes gait belt and helmet when ambulating due to being a fall risk.
HOUSING: Individual lives in a home in Mentor
DAY PROGRAM/EMPLOYMENT: Attends ADS 3x/week.
