DESCRIPTION OF REQUESTED SERVICES: Individual is looking for an HPC provider from 9am-6pm, 7 days/week. She requires total assistance with her medication, including glucometer, insulin, and other prescription meds. Also uses a CPAP machine that requires cleaning. Physical to total assistance with meal prep and planning. Physical assistance with cutting food and opening food containers. Total assistance with making appointments, medical and personal (hair). Physical assistance with cleaning up after BM. Physical assistance with showering. Physical assistance with household chores. Provide opportunities and transportation to community activities of interest. Physical assistance with stairs, curbs, uneven surfaces. Would also be beneficial if provider could serve as rep payee and help with budgeting, managing money.
