DESCRIPTION OF REQUESTED SERVICES: Individual is seeking a provider for Individual Employment Supports. He currently works in Eastlake, Ohio and his schedule is Wednesday-Saturday from 8:00am-4:30pm. His current job coach is no longer able to provide support but is willing to assist with a transition to a new provider. This individual needs monthly check-in support to ensure he continues to meet employer expectations and stays on task at work.

DESCRIPTION OF INDIVIDUAL: Individual is a 24-year-old male who lives in Kirtland with his parents. He works a job in the community, and is very friendly, kind and caring. He has been employed since March 2023 and overall is doing well. However, there are times he needs the support of a job coach to ensure he is meeting employer standards and maintain his success at work.
PROVIDER REQUIREMENTS: Provide all services as noted and discussed with the team. Since the family plans on utilizing FSS funding a provider would need to complete a W-9 to become a vendor with NEON for payment.
TRANSPORTATION NEEDS: N/A
DIAGNOSES: Mild Intellectual Disability, Fetal Alcohol Syndrome
HOUSING: Resides with family in Kirtland, OH.
DAY PROGRAM/EMPLOYMENT: Currently employed Wednesday-Saturday from 8:00am-4:30pm.
