DESCRIPTION OF INDIVIDUAL: Individual is a 34-year-old female diagnosed with Autism. Individual communicates verbally, is friendly, and gets along well with others. Individual likes staying physically active, shopping, and going out to eat. Individual requires support with emotional regulation, visits with mom, and supervision in the community.
PROVIDER REQUIREMENTS: HPC and HPC transportation
TRANSPORTATION NEEDS: non-modified vehicle.
