DESCRIPTION OF REQUESTED SERVICES: Seeking HPC services Monday-Friday from 3:45pm-7/8pm.
Provider would get individual off the school bus, hang out and interact with her, and feed her dinner. Assistance would include constant supervision, eating assistance, personal care (toileting and hygiene) assistance, and engagement in leisure activities. Individual is non-verbal–the provider should be comfortable with learning her non-verbal cues.

DESCRIPTION OF INDIVIDUAL: She is a fun and active 14-year-old. She is sweet, nonverbal, and emotionally intelligent, she utilizes Depends and would need assistance with personal hygiene. She rides in a wheelchair for long distances. She walks within her home and familiar settings with close supervision and assistance navigating stairs, curbs, or uneven surfaces. She needs close and constant supervision, and she doesn’t recognize or understand potential dangers in her environment, and she is curious and likes to explore. She should be closely supervised while eating to reduce the risk of choking or putting too much food in her mouth at once. She has a G-Tube for medication administration. When she is frustrated, individual may hit herself or others. She is a social butterfly and gives the best hugs!
PROVIDER REQUIREMENTS: She has emergency seizure medication (to be administered per her seizure protocol) which is used very infrequently as her daily seizure medication has her seizures under control. She receives her daily medications and supplemental hydration via G-Tube, but staff will not typically be administering medications. She is non-verbal, so the provider should be comfortable with learning her non-verbal cues.
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Cerebral Palsy, Pura Syndrome, Global Developmental Delays, Oral Apraxia, Silent Aspiration, Reflux, Cortical Vision Impairment, Hypotonia, Duplex Kidney, Myoclonus, Scoliosis, Mixed Receptive-Expressive Language Disorder, Movement Disorder, Anxiety, Seizures, Severe Intellectual Disability.
