DESCRIPTION OF REQUESTED SERVICES: Individual is a 39-year-old female seeking an HPC provider to participate in outings on Saturdays and Sunday’s, not to exceed 25h/month. Opportunity for additional hours as she recovers from knee surgery for the next few weeks, if desired.

DESCRIPTION OF INDIVIDUAL: This individual requires auditory supervision both at home and in the community. This individual is verbal and speaks English. Individual is independent with mobility. She enjoys talking with people and is very social. She is not always able to control her emotions/behaviors, she may demonstrate emotional outbursts including the use of foul language or using threatening language when upset or frustrated and needs reassurance from her staff to help manage her feelings. She has improved greatly with this over the past year. Having a sense of control in her life is important to her, she prefers to be offered choices. It is important to use a calm and friendly voice; demonstrate patience and ensure she feels that she is being heard when she is frustrated. She may need staff to remind her of strange danger and relationship boundaries. She is very activity driven and enjoys frequent outings to the Renaissance fair, 1899 in Willoughby, and Lake County Farm Park.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting. Training in verbal de-escalation could be beneficial but is not required.
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Mood Disorder, NOS, Mild Intellectual Disability, Fetal Alcohol Syndrome, Acid reflux, Migraines, Growths on Liver; Rheumatoid Arthritis and Gout.
HOUSING: Individual resides in an apartment in Mentor-on-the-Lake.
DAY PROGRAM/EMPLOYMENT: Individual works at Giant Eagle four days per week; currently not working due to recent knee surgery
