DESCRIPTION OF REQUESTED SERVICES: The individual’s guardian is looking for an agency to become the Representative Payee to manage Social Security and Medicaid benefits for the person and also provide Money Management services through the person’s waiver to ensure that all of the monthly bills are paid in a timely manner.
SEEKING AGENCY OR INDPENDENT PROVIDER: Guardian is seeking an agency provider.

DESCRIPTION OF INDIVIDUAL: The individual does understand the value of money and is able to ensure that he is receiving the accurate of change following the purchase of an item. He has demonstrated that he has the ability to manage up to $100 at a time. He is routinely receiving large amounts of money from his mother, which he receives via a cash app generally. He knows when the money in his cash app account is getting low and will reach out to his mother to let her know that he needs additional money to make purchases. He will also sell his belongings to people in his community and keep on his person any cash that he obtains from the sale of those items. He continues to work with his provider to understand how to budget because he will frequently spend all of his money at one time on something that he wants while forgetting about the things that he needs for his home or day-to-day life.
PROVIDER REQUIREMENTS: The Money Management provider must be able to maintain accurate records for reporting to Social Security, manage all the individual’s monthly bill paying to include, but not limited to, rent, household expenses and utilities. The provider will also need to manage all Medicaid redeterminations for the individual.

DIAGNOSES: Mood Dysregulation Disorder, ADHD, PTSD, Schizoaffective Disorder global type, Mild Intellectual Disability (FSIQ- 70), Autism Spectrum Disorder, Unspecified Anxiety Disorder, Depression, Reactive Attachment, Impulse Control Disorder, Bipolar Disorder, ODD, Asthma, Eczema, CNS Neoplasm, Possible Seizure disorder, pre-diabetic
HOUSING: Services will be provided to the individual at his home in Cleveland, Ohio.
DAY PROGRAM/EMPLOYMENT: Currently unemployed
