DESCRIPTION of INDIVIDUAL: Individual is a 60-year-old male who lives in Madison with his sister and brother-in-law. He is friendly and social. He enjoys spending time doing things in the community.
PROVIDER REQUIREMENTS: The provider will deliver ISP services as determined at the initial Service meeting.
DIAGNOSES: Moderate ID, degenerative joint disease of the lumbar spine, spinal stenosis of lumbar.
SUPPORTS NEEDED: Individual needs the use of his walker for long distances and support with stairs.
