DESCRIPTION OF INDIVIDUAL: 72-year-old female Individual currently receives shared living services with her sister. Individual would prefer to live with other females and have female staff when assisting with hygiene care. Individual is very sweet and friendly. She enjoys talking with others and having people around her. She enjoys playing with/holding toys throughout the day and listening to music.
PROVIDER REQUIREMENTS: Med cert 1. Individual can only navigate a few stairs and would need ground level bedroom access. She would benefit from ramp access to the home.
TRANSPORTATION NEEDS: Non-modified but may benefit from a modified vehicle in the future
DIAGNOSES: Moderate IDD, Legal Blindness, Lupus, Anxiety and Depression
HOUSING: Individual lives in Eastlake
