DESCRIPTION OF INDIVIDUAL: Individual is a 36-year-old male, who is very smart, prides himself on working in the community, being productive and accomplishing goals he makes for himself.
PROVIDER REQUIREMENTS: Job Coaching support: Assist with communication between managers and coworkers and to use a note pad to communicate if interpreter is not available. To assist with learning new tasks in job areas, and reminders about making sure all tasks are completed correctly. Offer guidance & support on appropriate vs non-appropriate work-related conversations/topics with others.
TRANSPORTATION NEEDS: NA
DIAGNOSES: Schizophrenia, OCD, Autism Spectrum, Deafness, Anxiety Disorder.
HOUSING: He lives at home in Mentor with two roommates
DAY PROGRAM/EMPLOYMENT: Employed 1-2 days per week, 4-5 hours per shift, and days of the week vary as to which days he works.
