DESCRIPTION of NEEDED SERVICES: Individual is looking for Residential Respite provider, for up to 7 hours on weekends; primarily Sundays, but may be flexible, mother currently provides shared living services and is looking for respite/support with ADL’s.

DESCRIPTION of INDIVIDUAL: Individual is a 25-year-old male. He uses a power wheelchair but can transfer with support. He requires visual supervision and constant monitoring, especially while eating. He is a very happy and kind young man. He enjoys singing and playing on his tablet. He enjoys going out to dinner and going to parks. He is friendly and outgoing. In the past he has displayed attention seeking aggression/self-injurious behaviors.
PROVIDER REQUIREMENTS: This provider will deliver ISP services as determined within the ISP and family.
DIAGNOSES: Lowe’s Syndrome, NOS, Anxiety, Legally Blind, Infantile Autism, Gastroparesis, GERD, Constipation, urinary retention, Glaucoma, diastasis high blood pressure, elevated liver enzymes and copper levels.
HOUSING: Individual lives with his mother and Sister in Eastlake
SUPPORTS NEEDED: Individual’s mom/guardian would like respite/assistance within home with ADL’s and outside of the home attending leisure/social activities.
