DESCRIPTION OF REQUESTED SERVICES: This is a request for a 21-year-old male seeking NMT to and from day program. Pick up from home at 9:15a, pick up from program at 2:00p.

DESCRIPTION OF INDIVIDUAL: This individual enjoys spending time watching his shows, listening to music on an iPad, and going for car rides out in the community. He is non-verbal and communicates through body language, gestures, vocalizations, and a speech generative device. He enjoys spending time in the car and going for walks in his adult push chair and riding his adaptive bicycle when available. The individual may engage in physical aggression or self-injury in times of frustration; change in routine, hygiene routines, difficulty communicating his wants/needs.
PROVIDER REQUIREMENTS: Provider must meet all necessary requirements as outlined in the ISP.
TRANSPORTATION NEEDS: non-modified vehicle.
DIAGNOSES: Global Developmental delays, ataxia, cerebral palsy, visual impairment, and seizure disorder
HOUSING: This individual currently lives by himself in a condo and receives 24-hour support from staff.
DAY PROGRAM/EMPLOYMENT: Individual currently attends OGHOSA program 5 days/week.
