DESCRIPTION OF REQUESTED SERVICES: Individual attends ADS M-F, outside of that, likely requires 24/7 supervision and assistance. CF needs total support with medication administration, transportation, cooking, home maintenance, shopping, purchasing, engagement in community activities, etc. CF can participate in chores, activities, shopping, cooking/baking, but needs guidance.

DESCRIPTION OF INDIVIDUAL: CF is very social and personable. She is good at communicating and loves engaging with others. Her faith and family are important to her. She enjoys making friends and being kind to others. She loves going out shopping, to ADS, outings, baking, etc. No mobility/physical limitations. No concerns associated with CF.
PROVIDER REQUIREMENTS: Medication administration twice per day. CF can take her meds when given to her. Her sister usually sets up a pill minder box for the month. Assistance with medical coordination needed (scheduling and attending appointments). No behaviors or major medical concerns.
TRANSPORTATION NEEDS: Non-modified vehicle.
DIAGNOSES: Mild ID
HOUSING: Lake County, currently searching for housing
DAY PROGRAM/EMPLOYMENT: Currently utilizes C&J Tender Heart in Lucas County, but looking to find an ADS and HPC in Lake County.
