DESCRIPTION OF REQUESTED SERVICES: Looking for a female HPC provider for 3-4 days a week from 4:00-8:00pm (family is flexible) and 1-2 days on the weekend from 12:00-4:00pm. Services for a 12-year-old female. Must be physically able to safely lift and assist with transfers of up to 80 pounds. Requires assistance with personal care, including changing diapers/briefs and providing total support with bathing and showers.

DESCRIPTION OF INDIVIDUAL: This individual is a 12-year-old female who resides in Willoughby Hills. She has a primary diagnosis of cerebral palsy and a history of seizures. Individual uses a power wheelchair for mobility and communicates verbally. She enjoys watching TV, hanging out with her siblings, shopping, and listening to music. Individual would like to be taken out into the community if possible (not required).
PROVIDER REQUIREMENTS: Provider needs to be able to manually lift and assist with transfers.
TRANSPORTATION NEEDS: Modified vehicle
