DESCRIPTION OF INDIVIDUAL: 33-year-old male individual looking for Voc Hab job tasks to prepare him for community work. OOD is involved currently. He has issues with hand dexterity and is looking for a program that can creatively support him to complete the tasks needed. His hope is to secure clerical job tasks to learn his strengths/things he needs to improve on. He is interested in data entry and has technology to support this (Laptop with Dragon app for talk to text capabilities). He is a very kind individual. He is laid back and enjoys sports, movies, video games & music.
PROVIDER REQUIREMENTS: Client requires transfers when using the restroom. Sit to stand equipment on site would be a bonus. He can eat independently but will need his items prepared for him.
TRANSPORTATION NEEDS: Requires non-modified vehicle. Will seek NMT once new program is secured.
DIAGNOSES: Cerebral Palsy, Mild ID, Generalized Anxiety, Major Depressive Disorder
HOUSING: Madison waiver home
DAY PROGRAM/EMPLOYMENT: Currently in facility-based Voc Hab program. Seeking community employment/a Voc site with more tasks available to him to fill his day.
