DESCRIPTION OF REQUESTED SERVICES: Looking for a respite provider for after-day program hours and overnights for a week in October and other dates in the future.

DESCRIPTION OF INDIVIDUAL: Individual is a 24-year-old male. He lives with his adoptive parents and family in Eastlake. He has Autism and schizophrenia, is highly verbal, likes to be busy, and plays miracle league baseball. He just started a new day program. He is friendly and social most of the time. However, there are times that he can be moody and break toys or throw things.
PROVIDER REQUIREMENTS: A provider would need to be able to deliver all ISP services as determined at the ISP meeting.
TRANSPORTATION NEEDS: Non-modified transportation
DIAGNOSES: Autism and schizophrenia
HOUSING: Individual lives with his family in Eastlake.
DAY PROGRAM/EMPLOYMENT: Individual attends an ADS program in Mentor Monday through Friday.
