DESCRIPTION OF NEEDED SERVICES: Individual is in need of HPC services in the community. On the weekends and one evening per week. Individual will require transportation to and from the outings.

DESCRIPTION OF INDIVIDUAL: The individual enjoys being in the community specifically visiting different parks, eating at restaurants, bowling, and taking walks. Individual requires a wheelchair while in the community.
PROVIDER REQUIREMENTS: Individual will not need any medications while in the community.
TRANSPORTATION NEEDS: Modified vehicle
DIAGNOSES: Intellectual disability and Cerebral Palsy,
HOUSING: Individual lives with one other person in Mentor, Ohio in a group home. Individual is looking to receive services in the community.
DAY PROGRAM/EMPLOYMENT: Individual attends a day program M-F.
